Decide if you want the physicians' names or the address printed under the headline.

Date to Call: Enter a time frame that you feel is sufficient for you to get the results and put them on the system (example 7-14 days). You can also leave it blank and write the date you want the patient to call.

Medical Record Number: This number may vary in size from 4 to 15 digits. It should coincide with your office patient identification system. For example a social security number or any other universal office identification will work as a medical record number. This field can be left blank and you can write the number for the patient. If you are planning on using the patient's social security number, you can write "Your Social Security Number".
